Applicant eligibility will depend on the particular grant program under which services are being requested. All applicants must participate in an intake and assessment interview(s) to determine eligibility, need and potential resource availability. Written documentation of eligibility from appropriate and/or approved sources must be provided prior to receiving grant-related services. Documentation includes, but is not limited to, verification of HIV status and verification of all individual and household income.
HIV Care (Ryan White)
- Any individual with a HIV positive or AIDS diagnosis are eligible to receive HIV Care services (refer to matrix of supportive services).
- Eligibility is not income based.
- These funds are payor of last resort.
- Service priority is given to case management, medication and medical assistance (HIV related issues only).
- All services are dependent upon funding availability.
HOPWA (Housing Of People With AIDS)
- An individual must have a HIV positive or AIDS diagnosis.
- Income meets the low income definition established by HUD (less than 80% of the mean income).
- Family members of eligible individuals may also receive services

SHP (Supportive Housing Grant)
- An individual must have a HIV positive or AIDS diagnosis.
- Income meets the low income definition established by HUD (less than 80% of the mean income).
Meets the homelessness definition established by HUD. Homelessness status requires verification from a shelter or social services or law enforcement agency. Imminent danger requires documentation including, but not limited to, an eviction notice, letter from other involved party, etc. Substandard housing may be established by an HQS inspection.
- Family members of eligible individuals may also receive services.
- All services are dependent upon funding availability.
Shelter Plus Care (S+C)
- An individual must have a HIV positive or AIDS diagnosis.
- Income meets the low income definition established by HUD (less than 50% of the mean income).

Meets the homelessness definition established by HUD. Homelessness status requires verification from a shelter or social services or law enforcement agency. Imminent danger of homeless or substandard housing conditions do not meet the homelessness requirement for S+C services.
- Must have a documented disability.
- Family members of eligible individuals may also receive services.
- All services are dependent upon funding availability.
Definitions
Family is defined in the HUD regulations as a household of two or more related persons. The term family also includes one or more eligible persons living with another person or persons who are determined to be important to their care or well being, and the surviving member or members of any family described in this definition who were living with the individual with HIV/AIDS, in a Four A’s subsidized unit, at the time of that individual’s death.
