Purpose
- Support services are dependent on available funding.
- Supportive services may differ dependent on the funding source.
- Clients are required to meet eligibility criteria for each support services dependent on the program/funding source providing the service.
- All support services provided to a client must be supported by their Individual Wellness Plan and must assist the client in maintaining a healthy lifestyle.
- In case of inadequate funding, priority services will be funded. Priority services include case management, medical care, medication, housing, and transportation. Funding for other services will be evaluated on a case-by-case basis.
Types of Supportive Services
- Case Management: All clients at the Four A’s will receive case management services.
- Medical Care & Assistance: Available on a limited basis for clients who have been identified as having no other resources and/or in situations where all other alternatives have been exhausted. Does not pay for in-hospital care or extensive treatments (i.e. chemotherapy).
- Dental Care: Emergency dental care (defined as dental care which is necessary to prevent or treat conditions which compromise health, particularly in the area of nutrition and potentially systemic infection) will be available on a limited basis and only with an approved dental care clinic. Preventative dental cleaning can also be paid if no insurance or financial ability is available.
- Optical Care: The purchase of glasses may be available for those clients who require corrective lenses to perform daily living tasks, and for whom no other resources exist.
- Alternative/Holistic Medicine: Costs can be covered, but there will need to be a treatment plan outlined and only pre-approved amounts will be paid.
- Education & Support Groups: HIV/AIDS related education and peer support opportunities will be available to clients at the Four A’s and/or other sites as appropriate and subject to funds available.
- Education, Rehabilitation/Vocational Training & Self-Sufficiency: Educational assistance both in payment and in planning will be available to clients for the purposes of increasing vocational skills and/or increasing self-sufficiency. This assistance will be evaluated on a case-by-case basis and all other resources available will be accessed before grant funds are made available. This assistance will pay only for tuition, registration costs, and required supplies (i.e. books, tools).
- Chemical Dependency: Four A’s works with local treatment facilities to facilitate treatment and other support services for clients who have substance abuse problems. Treatment fees and other payments for substance abuse treatment services will be negotiated on a case by case basis and advocacy services will be provided by Four A’s staff. On site Substance Abuse counselor is also available.
- Home Health Care Services: Available to clients requiring these services, but who have no ability to pay for (or no benefits to cover) such services. The need for home health care services is identified by the attending physician. Maximum amount of $1,000.00 per month.
- Mental Health: Mental health services will be available for those clients in need of these services. In most cases, Four A’s will have an approved practitioner to whom the participant will be referred.
- Transportation: Available to clients for the purposes of obtaining medical care and attending necessary case management/housing appointments. This assistance will include rides in the Four A’s agency van, the purchase of a bus pass and/or bus tokens, and payment for taxi cabs. Priority for this assistance will be given to clients with poor health and/or limited transportation options.
